Financial Policy
If we participate in your health plan (please inquire) or if you have out of network benefits, we may bill your insurance company directly, after receiving payment from you for any applicable co-payment and deductible at the time of your visit. If we do not contract with your insurance, you will be responsible for payment in full at the time of your service (your insurance company may reimburse you directly). In the event your insurance determines a service to be “not covered,” you will be responsible for the fee. We will send you an initial “Statement of Balance Due” free; on all future statements, at $10.00 processing fee will be added. We reserve the right to charge interest on overdue balances and to charge for collection and/or legal fees. A fee of $35.00 will be charged on any check returned by the bank for insufficient funds.

Missed or Late Appointments
There is a $50 charge for any appointments not cancelled at least one day prior to the scheduled visit. Please call us in advance and we will do our best to reschedule your appointment. Should you arrive late for your appointment or if the office is running late, you may have to wait to be seen or we may need to reschedule your appointment to a later date.

Release of Protected Health Information for the Purposes of Treatment, Payment and Practice Options
“Protected Health Information” means information collected from you and created or received by Facial Plastic, Reconstructive and Laser Surgery, PLLC. This protected health information relates to your past, present or future physical or mental health or condition and may identify you. Our “Notice of Privacy Practices” is a booklet written in plain language that describes the types of uses and disclosures of your protected health information, and also describes your rights with respect to your protected health information.
